60 minute visit. Includes a one on one meeting with Dr. Perez to discuss thorough medical history and current concerns as well as a full neurologic and comprehensive exam, differential diagnosis, and initial plan of care. She will review any previous records with you in person.
30 minute visit. Follow up visits include a meeting with Dr. Perez to review lab, testing, and imaging results as well as a neurological evaluation and discussion. She will review any images with you and provide an independent read in addition to the official radiology report. The plan of care may be optimized or adjusted depending on the results reviewed during this visit.
A brief visit includes any visit that requires less than 20 minutes of time. This may include a discussion about medication tolerance, side effects, results of basic labs, review of medication interactions, or other topic of concern to discuss with Dr. Perez. Plan of care may be optimized at this time.
Established patients may see Dr. Perez for maintenance visits via Telehealth. If she determines that the patient will need an in person evaluation, then this will be discussed with the patient.
Patients may see Dr. Perez for a procedure that is provided in the office.
Established patients can request for Dr. Perez to fill out various forms that may be related to handicap/ADA/Accessibility needs, nursing home applications, disability, and other related forms.
Dr. Perez performs wheelchair/mobility evaluations for patients requiring specialized power chairs for optimal independence and mobility. These visits can be conducted in office or virtually.
EEG testing is conveniently provided in house. A technician will perform the EEG in the office and Dr. Perez will provide an official review and report of the testing as well as recommendations based on the results of the test.
